Photo & Media Release I give permission for TAL: The Abilities Lounge to take photographs and/or video recordings of the participant listed above during this event. I understand that these images and recordings may be used for promotional, marketing, and educational purposes, including but not limited to social media, website content, printed materials, and advertisements. I understand that no personal identifying information such as full name, or address will be shared. Images will be used respectfully and in alignment with TAL’s mission of inclusion and empowerment.

Waiver of Liability

I understand that participation in activities at TAL: The Abilities Lounge may/may not involve some level of risk. By signing this form, I agree to assume responsibility for myself while engaging in activities and release TAL, its owners, and staff from liability for any injuries or incidents that may occur during participation. This facility uses video surveillance for safety and security. By entering, you consent to being recorded.
