Excused Absence Request
Please let us know the 1 date you would like to request an excused absence for as well as the reason. Requests MAY NOT exceed 1 absence. If there are no dates your dancer would need to be excused from please do not answer this question.

Waivers
Do you understand that there is a MANDATORY parent meeting during your child's audition time?
This parent meeting will take place in the 3rd studio at SDSD.
Do you understand that this form and the $100 Audition fee must be submitted prior to or at the time of audition to be considered for SDDT's Nutcracker?
Remember that your audition fee covers your child's costume, t-shirt, rehearsal time and choreographer. This fee must be submitted online via PayPal on the next page of this form.
Waivers

The following waivers must be signed for participation
I, the undersigned, give my permission for the above mentioned dancer to participate in SDDT's Nutcracker audition process, rehearsals, and performances. I recognize the possibility of physical injury and risk of illness to the student associated with taking part in this activity. Upon signing this waiver, release and forever discharge any and all rights and claims for damages, which may arise now or in the future against SDDT and its assigned representatives or volunteers from any and all liability and for any and all damages, illness and/or injuries which may be sustained or suffered by the student while participating in the SDDT Nutcracker Audition, rehearsals or performances. Furthermore, in the event SDDT is unable to contact the authorized person(s) on this form, I the parent or legal guardian, hereby give my consent for emergency medical care. Transportation to the hospital will be at the discretion of the Emergency Technicians on site.

This waiver is REQUIRED for participation.
I, the undersinged, hereby grant to the South Dayton Dance Theatre all rights of every kind to use my child's photographs or films. I agree that SDSD and SDDT has the right to record the material for use on all formats now known and yet to be discovered. I agree to indemnify and hold SDSD and SDDT, its agents and officers harmless against liability associated with the distribution and/or transmission of any recorded media. I fully understand that all above recorded materials may be used now and in the future by the South Dayton School of Dance or South Dayton Dance Theatre.
