Please complete this registration form for all team coaches attending the 2026 USTA National Congress. Early registration is encouraged. Note: All 2025–26 memberships expire July 31, 2026. 2027 Team & Coach Memberships must be paid by September 1, 2026 to be eligible to vote. Teams with less than two concurrent USTA Memberships will not be eligible to vote at the USTA Congress Meeting, but will have a voice. For questions, refer to the official event information.
