1. Acknowledgement of Risk
I acknowledge that participation in basketball and related athletic activities involves inherent risks, including but not limited to:
Slips, trips and falls
Collision with players, coaches, officials or spectators
Contact with basketballs, equipment or facility structures
Muscle strains and sprains
Broken bones
Concussions
Head injuries
Dental injuries
Permanent disability
Death
I understand that these risks may occur despite reasonable supervision and safety procedures.
I voluntarily accept all risks associated with participation.
2. Medical Fitness
I certify that my child is physically able to participate in basketball activities.
I have disclosed all known:
medical conditions
allergies
medications
behavioural concerns
physical limitations
that may affect participation.
I understand it is my responsibility to update B-Ballerz Athletic Academy if any medical information changes.
3. Emergency Medical Treatment
If I cannot be reached during an emergency, I authorize B-Ballerz Athletic Academy, B-Ballerz Elite Academy, its employees, coaches, volunteers or representatives to obtain emergency medical treatment for my child.
I understand that all associated medical expenses remain my responsibility.
4. Release of Liability
To the fullest extent permitted by Ontario law, I release and forever discharge:

B-Ballerz Elite Academy
Owners
Directors
Coaches
Assistant Coaches
Volunteers
Employees
Contractors
Facility Owners
Schools
Recreation Centres
Municipalities
Landlords
Event Hosts
Sponsors
from any claims arising from injury, illness, property damage, disability or death resulting from participation, except where caused by gross negligence or willful misconduct.
5. Assumption of Risk at Any Location
I understand that B-Ballerz programs may be conducted at:
B-Ballerz Athletic Academy
B-Ballerz Elite Academy
Schools
Recreation Centres
Community Centres
Municipal Gyms
Outdoor Courts
Parks
Tournament Venues
Rental Facilities
Any future training location selected by B-Ballerz
This waiver applies regardless of where the activity occurs.
6. Competitive Basketball
If my child joins a competitive team, I understand participation may include:
Practices
House League
League Games
Exhibition Games
Tournaments
Team Events
Team Meetings
Strength & Conditioning
Off-site activities
League fees, tournament fees, uniforms and travel expenses may be charged separately.
7. Transportation
I understand that parents remain responsible for transportation unless otherwise communicated.
If B-Ballerz organizes transportation for a specific event, I authorize my child to participate.
8. Photography & Media Release
I grant permission for B-Ballerz Athletic Academy and B-Ballerz Elite Academy to photograph and record my child during programs.
These images and videos may be used for:
Website
Instagram
Facebook
YouTube
Promotional materials
Advertising
Printed materials
Educational purposes
9. Behaviour Policy
I understand that respectful behaviour is expected from:
athletes
parents
siblings
guests
Abusive, threatening, discriminatory or inappropriate behaviour may result in removal from the program without refund.
10. Personal Property
B-Ballerz is not responsible for:
lost property
stolen property
damaged property
including phones, wallets, jewellery, clothing or sporting equipment.
11. Attendance
I understand that attendance is important for athlete development.
Missed sessions are not refunded except where required by law or under the academy’s published policies.
12. Payment & Membership
I acknowledge that I have read and agreed to the academy’s:

Membership Agreement
Cancellation Policy
Commitment Policy
These documents form part of this agreement.
13. Severability
If any part of this agreement is found unenforceable, the remaining portions remain in effect.
14. Governing Law
This agreement shall be governed by the laws of the Province of Ontario and the laws of Canada applicable therein.
15. Parent/Guardian Declaration
I certify that:
I am the legal parent or guardian of the participant.
I have carefully read this entire agreement.
I fully understand its contents.
I understand I am giving up certain legal rights by signing this document.
I voluntarily agree to all terms on behalf of myself and my child.
