PeeWee
Must be in 2nd Grade or 3rd Grade in 2026-27 School Year.

Junior
Must be in 4th Grade or 5th Grade in 2026-27 School Year.

Middle School
Must be in Middle School (Grades 6, 7, 8) in 2026-27 School Year.

High School
Must be in High School (Grades 9, 10, 11, 12) in 2026-27 School Year.
Cannot turn 18 years old before 8/1/2026

Players will have the opportunity to be packaged up as pairs of no more than 3 when teams are being formed. After that, teams will be evenly placed together. You may leave in the comment section at the end of registration the names of the players you wish for your child to be paired with. We cannot guarantee that your child will be paired with the players of your interest, but we will try our best.

RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AND PARENTAL CONSENT AGREEMENT ("AGREEMENT") IN CONSIDERATION of being permitted to participate in any way in athletic activities (the "Activities") I, for myself for personal representatives, assigns, heirs, and next of kin: 1. ACKNOWLEDGE, agree, and represent that I understand the nature of the Activities and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the Activity. 2. FULLY UNDERSTAND THAT: (a)THE ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH ("RISKS"); (b) these Risks and dangers may be caused by my own actions or inaction’s, the actions or inaction’s of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISK AND SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation or that of the minor in the Activity. 3. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE GRIDIRON SPORTS GROUP, their respective administrators, directors, agents, officers, members, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owner and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS, INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child/ward’s involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the Releases, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim. If this is not completed, your child will not be covered by their insurance policy leaving you responsible for any medical expenses incurred. 4. ACKNOWLEDGE, agree, and represent that I understand GRIDIRON SPORTS GROUP, is not responsible for loss or damage to personal possessions and that my/my child's photograph or video may be used in future promotions. 5. I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child/ward’s readiness for participation and/or in the program itself, I will remove my child/ward from the participation and bring such attention of the nearest official immediately 6. If a player is injured at a league sponsored event (practice, games etc) It is the players responsibility to immediately report it to their coach so they can be evaluated to determine if medical assistance is needed. Parents should then follow up with their coach if medical assistance is needed and a league incident form should be filed. It is the participant / parents responsibility to ensure that this form is submitted to the league. I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT / ELECTRONICALLY ACKNOWLEDGING IT AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.

MINOR RELEASE - AND I, THE MINOR’S PARENT AND/OR LEGAL GUARDIAN, UNDERSTAND THE NATURE OF THE ACTIVITIES AND THE MINOR’S EXPERIENCE AND CAPABILITIES AND BELIEVE THE MINOR TO BE QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL CONDITION TO PARTICIPATE IN SUCH ACTIVITY. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS EACH OF THE RELEASEE’S FROM ALL LIABILITY CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON THE MINOR’S ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATION AND FURTHER AGREE THAT IF, DESPITE THIS RELEASE, I, THE MINOR, OR ANYONE ON THE MINOR’S BEHALF MAKES A CLAIM AGAINST ANY OF THE RELEASEES NAMED ABOVE, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE, OR COST ANY MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.

PHOTO/VIDEO RELEASE- I hereby give my consent for Gridiron Sports Group to use my Childs photograph and or action videos and likeness to be used in its publications, including its website, youtube, facebook and any other form of public media. I release them from any expectation of confidentiality for the undersigned minor children and myself and attest that I am the parent or legal guardian of the children listed on this registration form. I hereby authorize Gridiron Sports Group to publish the photographs, videos and sounds taken of the registered player for the activity registered for, and their name for use in the Gridiron Sports Group printed publications and website. I also authorize the use of any likeness in gaming, apps or any marketable type item. Any pictures or videos taken by me will not be sold and are copyright by Gridiron Sports Group I acknowledge that since my participation in publications and websites produced by Gridiron Sports Group is voluntary, I will receive no financial compensation. I further agree that my participation in any publication and website produced by Gridiron Sports Group confers upon me no rights of ownership whatsoever. I release Gridiron Sports , its contractors, volunteers and its employees from liability for any claims by me or any third party in connection with mine or my childs (registrant’s) participation

CODE OF CONDUCT - ALL GSG Players, coaches and spectators must adhere to the Code of conduct set forth by GSG. Upon registering your child a code of conduct form was completed either on paper or electronically. GSG Follows the NJ Model Athletic Code of conduct as a baselineAny verbal or physical altercations of adults will result in an immediate season suspension of coaching and observing their childs games. Any child involved with this type of behavior is subject to the same penalty. This also includes social media, emails or any occurrence that is deemed unacceptable by GSG. Any form of sexual harassment or misconduct will also fall under these guidelines. A decision involving any violation of the above stated guidelines will be investigated and a decision voted on by the Gridiron Sports Group Board within 72 hours. The participant will then be notified of the decision either electronically or verbally.

UNDERSTANDING OF RISK - I understand the seriousness of the risks involved in participating in this program, my personal responsibilitiesfor adhering to rules and regulation, and accept them as a participant.I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT or ELECTRONICALLY ACKNOWLEDGE AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
