By signing this application, I certify that the above information is correct, that I am financially responsible for any outstanding balance, and that I have received, read, and agree with the Student Handbook, the Code of Conduct, and the 16 Fundamental Truths of the Assemblies of God. I AUTHORIZE the Spiritual Formation Center,to provide any necessary information to process my certificate/diploma or special recognition and/or photograph for publication. The Spiritual Formation Center solely provides Christian education services. We are not responsible for any other services provided by affiliated extension sites.

*Program tuition consists of seven (7) courses, each with fifteen (15) academic hours. The application fee and all payments made toward courses included in the annual enrollment are strictly non-refundable. However, under exceptional personal circumstances, payments may be transferred to another student, subject to prior written approval by the program administration.

Acceptance of Payment Agreement Terms

By signing this Payment Agreement, the student acknowledges and agrees that they are entering into an academic and financial commitment to complete the seven (7) courses required during the academic year for the selected program. Tuition and payments established in this agreement guarantee the student’s participation in the program and access to the academic resources provided by the Spiritual Formation Center.The student agrees to make all payments on the scheduled dates according to the selected payment option. Failure to make payments may affect participation in the program, access to courses, instructional materials, academic platforms, and other associated benefits.All payments made, including the application fee, tuition, installment payments, monthly payments, and any other program-related charges, areSTRICTLY NON-REFUNDABLE. This policy applies regardless of attendance, participation, voluntary withdrawal, program abandonment, changes in personal circumstances, or any other situation that prevents the student from completing the courses.In exceptional and properly documented circumstances, payments already made may be considered for transfer to another student only with prior written approval from the program administration. Such a transfer does not constitute a refund and does not guarantee approval of the request.By signing this document, the student declares that they have read, understood, and fully accepted the terms of this agreement, including the NON-REFUNDABLE payment policy, and commits to completing the seven (7) required courses and complying with the payment schedule corresponding to the selected payment option.

Student Vow

To maintain the School of Ministry as a spiritual institution, worthy of vocation and faithful to the biblical principles, I commit myself to:
1. Maintain my spiritual life in such a way that it is a faithful example to my fellow students through my testimony, conversation, and devotional life.
2. Be an active member of a church and a believer of good standing.
3. Strive to draw closer to God through diligent study of the Holy Scriptures (Bible); studying and searching reference books and other sources that can help me.
4. Use my capacity and skills to the fullest to prepare all assigned work, by avoiding the use of plagiarism (imitating someone else’s work, giving it as my own) and artificial intelligence (AI). Understanding that the use of plagiarism and artificial intelligence (AI) will not be tolerated and its consequences are suspension or expulsion from the School of Ministry.
5. Help in the activities of the Branch and encourage my colleagues to cooperate in them.
6. Be punctual in attending each class, the devotional, and any other activity sponsored by the Branch.
7. Not to violate the rules and standards of conduct in the classroom.
8. To be punctual in fulfilling my financial responsibilities with the institution.
9. Do not interrupt the class with controversial questions or polemics that might create a bad environment for my fellow students.
10. Be obedient and submit to the staff of the Branch and my teachers; and respect my fellow students.
11. Read and comply with everything stipulated in the Student Manual.
12. Accept and submit to the 16 Fundamental Truths which is based on the doctrine of the Assemblies of God.
