Please complete one form per church. Be sure to include ALL persons wanting a lunch-students AND leaders.
The bagged lunches are a seperate order from Campwise registrations. These will be paid for seperately from any Campwise invoices. You can pay by card directly on this form at the time of ordering, or you can select to pay by check. Be sure and follow the prompts below.
