Mealtime Magic: Food Exploration Groups is an educational, parent-participatory program facilitated by licensed Speech-Language Pathologists. It is not a clinical feeding therapy service and does not replace individualized evaluation or treatment. By registering, I understand and acknowledge that food and oral-sensory activities are involved, and I accept full responsibility for monitoring my child/children during all food exploration activities. I confirm that I have disclosed all known food allergies, dietary restrictions, feeding diagnoses, swallowing difficulties (including dysphagia or aspiration history), and any physician- or therapist-recommended dietary modifications. I understand that children with significant swallowing concerns, a history of aspiration, or medically complex feeding needs may not be appropriate candidates for this program, and I have consulted with my child/children's medical provider or feeding therapist as needed prior to enrollment. MJ Kidz LLC and We Rock the Spectrum Gloucester assume no liability for adverse reactions, choking incidents, gagging, sensory responses, or any injury arising from participation in food exploration activities. A parent or legal guardian must be present and actively supervising their child/children throughout the entire session. I have read and understand this statement and voluntarily consent to my child/children's participation.
