LIABILITY WAIVER, RELEASE, AND MEDIA CONSENT AGREEMENT
Facility Name: Friga Facility
Associated Parties: Dvontay Friga, Friga Facility staff, coaches, trainers, employees, contractors, volunteers, and affiliates (collectively referred to as “Released Parties”)
ASSUMPTION OF RISK
I understand that participation in physical activities, training, workouts, events, or any use of the Friga Facility involves inherent risks, including but not limited to bodily injury, illness, permanent disability, or death. I voluntarily choose to participate and fully assume all risks associated with my participation.
RELEASE OF LIABILITY
I hereby release, waive, and discharge Friga Facility, Dvontay Friga, and all Released Parties from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, accident, illness, or damages that may occur while participating in activities at or associated with the Friga Facility, whether caused by negligence or otherwise.
PERSONAL PROPERTY
I understand that Friga Facility and the Released Parties are not responsible for any lost, stolen, or damaged personal property, including but not limited to clothing, equipment, phones, jewelry, or other valuables.
NO ACCOUNTABILITY CLAUSE
I agree that Friga Facility, Dvontay Friga, and the Released Parties hold no accountability or responsibility for any other situations, incidents, or outcomes that may occur during my participation, except where prohibited by law.
MEDIA RELEASE & CONSENT
I grant Friga Facility, Dvontay Friga, and the Released Parties the irrevocable right and permission to photograph, video record, and otherwise capture my likeness, voice, and appearance during activities at the facility.
I further grant permission for these recordings to be used, edited, published, posted, and distributed in any form, including but not limited to social media, websites, promotional materials, and advertisements, without compensation or prior approval. I waive any right to inspect or approve the final content.
ACKNOWLEDGMENT & AGREEMENT
I confirm that I have read and fully understand this agreement. I understand that by signing this document, I am giving up certain legal rights. I sign this freely and voluntarily.
FOR MINORS (UNDER 18 YEARS OF AGE)
I am the parent or legal guardian of the participant listed above. I consent to their participation and agree to all terms of this waiver on their behalf.
