🏀 O-Town H.U.S.T.L.E Basketball
Liability Waiver, Assumption of Risk, Medical Authorization, and Media Release
I, the undersigned parent/legal guardian of the participant identified in this registration, hereby acknowledge and agree to the following:
Assumption of Risk
I understand that participation in basketball training, conditioning, drills, scrimmages, competitions, and related athletic activities involves inherent risks, including but not limited to falls, collisions, physical contact, overexertion, equipment-related injuries, illness, serious bodily injury, permanent disability, paralysis, and death.
I voluntarily assume all risks associated with my child's participation in the O-Town H.U.S.T.L.E Elite Skills Camp and any related activities.
Release of Liability
In consideration for being permitted to participate in the O-Town H.U.S.T.L.E Elite Skills Camp, I, on behalf of myself, my child, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue:
- O-Town H.U.S.T.L.E
- Coach Mike Freeman
- Ganon Baker
- All camp coaches, trainers, instructors, volunteers, and staff
- Event organizers and sponsors
- Facility owners, operators, and employees
Any affiliated organizations, representatives, agents, successors, and assigns from any and all claims, liabilities, demands, damages, losses, costs, expenses, actions, or causes of action arising from or related to participation in the camp, including those resulting from ordinary negligence to the fullest extent permitted by law.
Medical Authorization
I certify that my child is physically able to participate in basketball activities and has no medical condition that would prevent safe participation, except as disclosed during registration.
In the event of injury, illness, or emergency, I authorize camp staff to obtain emergency medical treatment for my child if I cannot be reached immediately.
I understand that I am solely responsible for any medical expenses incurred as a result of participation in camp activities.
Insurance Responsibility
I understand that O-Town H.U.S.T.L.E, Ganon Baker, and all associated staff do not provide medical or accident insurance coverage for participants.
I certify that my child is covered by personal medical insurance and accept full responsibility for any medical costs incurred.
Code of Conduct
I understand that athletes are expected to demonstrate sportsmanship, respect, coachability, and appropriate behavior throughout the camp.
O-Town H.U.S.T.L.E reserves the right to dismiss any participant whose behavior is deemed unsafe, disruptive, abusive, or detrimental to the camp environment without refund.
Photo & Media Release
I hereby consent, without further consideration or compensation, to the use (full or partial) of all photos, audio, or video recordings made of my child or family members during O-Town H.U.S.T.L.E events. These materials may be used for documentation, promotional content, or web-based media. I further release O-Town H.U.S.T.L.E and its volunteers from any liability that may arise from the use of such media.
