Injury Risk Acknowledgment & Waiver
Please read the following information carefully. By signing this form, you acknowledge the inherent risks of martial arts training and agree to the terms of participation for your child(ren).
Nature of the Activity: I understand that Brazilian Jiu-Jitsu (BJJ) and martial arts are close-contact combat sports involving grappling, wrestling, takedowns, and physical exertion.
Assumption of Risk: While Drop Bear BJJ prioritizes safety, proper technique, and qualified supervision, I acknowledge that there is an inherent risk of physical injury. These risks include, but are not limited to, minor injuries (eg. bruises, scrapes, muscle strains etc.) as well as more significant injuries (eg. ligament/tendon injuries, fractures, or concussions etc.). I voluntarily assume all risks associated with my child’s participation.
Medical Fitness & Disclosure: I certify that my child is physically capable of participating in this program. I have fully disclosed all relevant medical, physical, or neurodiversity conditions in the fields provided above.
Emergency Medical Treatment: In the event of an injury or medical emergency, I authorize the staff of Drop Bear BJJ to seek appropriate medical attention or ambulance transport for my child if I cannot be reached immediately. I agree to cover any associated medical costs.
Release of Liability: I hereby release, waive, and hold harmless Drop Bear BJJ, its owners, instructors, staff, and volunteers from any and all liability, claims, demands, or causes of action arising out of any injury, loss, or damage sustained by my child during the school holiday program.
By providing your signature below, you confirm that you are the parent or legal guardian of the registered student(s), and that you have read, understood, and agreed to this waiver.
