Camp Registration

Participant Release of Liability Waiver. 1) I hereby acknowledge and understand that participation in sports and physical activities involves inherent risks of injury. I am voluntarily participating in such activities with Shoot Like A Girl. I hereby assume full responsibility for any and all risks, injuries, or damages that may occur as a result of my participation. 2) I release, waive, discharge, and covenant not to sue Shoot Like A Girl, its owners, employees, instructors, and agents from any and all liability for any injury, loss, damage, or expense arising from my participation in activities with these camps and clinics. 3) I understand that this waiver applies to all activities offered by Shoot Like A Girl, including but not limited to basketball training .4) In the event of an emergency, I authorize the Released Parties to obtain medical treatment for me if deemed necessary, and I agree to be responsible for any costs associated with such treatment. 5) I acknowledge that I have carefully read and fully understand this waiver and voluntarily sign it as my own free act and deed. I further agree that this waiver is binding upon my heirs, executors, administrators, and assigns. 6) I also acknowledge that photographs and video may be taken of me in my participation in, and attendance at camps or clinics, and hereby freely agree to allow without restriction all uses of such photos and videos in the reporting of club activities, and/or in the promotion of such, its location, other sporting events, sports in general, and/or related purposes. SHOOT LIKE A GIRL Cancellation Policy: If you need to cancel for any reason up until 24 hours prior to camp check-in, you will receive a camp credit voucher for tuition fees paid. After the 24-hour cancellation window has expired, no refunds or vouchers for camp fees paid will be issued for voluntary withdrawal, schedule conflicts, or behavioral dismissal. Participants who DO NOT attend their registered session (no-shows for any reason) or who do not contact Shoot Like A Girl to cancel at least 24 hours prior to camp check-in will not receive any voucher or refund. Vouchers will be considered for cancellations due to family emergencies, illness, or injury upon receipt of required documentation such as physician's notes, medical records, etc. By signing I acknowledge that I HAVE READ THIS RELEASE OF LIABILITY, ASSUMPTION OF RISK AGREEMENT AND THE CANCELLATION POLICY, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT,AND SIGN IT FREELY.
