Liability and Waiver:
Release from Liability: Recognizing that the Islamic Center of Naperville (ICN) will do its best to ensure a safe experience, I understand that accidents may occur from my participation and my doubles partner's participation in the tournament. We agree to assume these risks. By signing below, I, on behalf of myself and my doubles partner, release ICN and the ICN Program organizers from all liability based on any damage, loss or injury whether it is the result of ordinary negligence or otherwise, caused to me or my doubles partner, from participation in the tournament.
Medical Treatment Authorization: We, the participants in the Men's Doubles Badminton Tournament, hereby authorize and consent to the provision of medical treatment by qualified medical personnel and to transportation for medical evaluation and treatment as deemed necessary. We further authorize the disclosure of our relevant health information and consent to any medical, surgical, hospital, or emergency care, treatment, and procedures, including but not limited to the administration of anesthetics, diagnostic tests, X-ray examinations, transfusions, injections, medications, and other necessary medical interventions. Such treatment may be rendered to either or both of us by a licensed physician, medical provider, or hospital selected by the tournament organizers when, in the professional judgment of the attending physician or medical personnel, such treatment is considered necessary or advisable to protect our health, safety, or well-being. We acknowledge and agree that this authorization shall remain in effect for the duration of the tournament and related activities.
I have read and understand the above and have completed this form to the best of my ability on my behalf and my doubles partner's behalf for the Men's Doubles Badminton Tournament. The participants will abide by all Islamic values and morals & rules of the tournament. There will be a zero tolerance policy for non-compliance of any un-Islamic behavior, bullying or disrespect. Any violation of such may result in a termination of the participant(s) from the tournament without reimbursement.
