I grant permission for my child(ren) to participate in the OLPH Faith Formation Program. As a parent and legal guardian, I remain legally responsible for any personal actions taken by the minors listed above. I agree on behalf of myself, my child(ren) named herein, or our heirs, successors, and assigns, to hold harmless and defend Our Lady of Perpetual Help Parish and the Roman Catholic Bishop of San Diego, and their respective clergy, its officers, directors, employees, and agents, chaperones, representatives or volunteers associated with the activities, from any claim arising from or in connection with my minors) attending the activities or in connection with any illness or injury including death or cost of medical treatment in connection therewith, and I agree to compensate Our Lady of Perpetual Help Parish, its officers, directors, and agents, and the Roman Catholic Bishop of San Diego, and their respective clergy, its employees and agents and chaperones, or representatives associated with the activities for reasonable attorney's fees and expenses which may incur in any action brought against them as a result of such injury or damage, unless such claim arises from the negligence of Our Lady of Perpetual Help Parish or the Roman Catholic Bishop of San Diego. I grant permission for Our Lady of Perpetual Help Parish, their respective employees, representatives, or volunteers, to photograph or record on audio and video (tape or digital) my child(ren) to further the mission of Our Lady of Perpetual Help Parish. Photos, audio, or video may only be used in printed materials and any other visual display or media sponsored by Our Lady of Perpetual Help Parish. I understand that such photos and video recordings will be used for Our Lady of Perpetual Help Parish-related purposes only and will not be used for any commercial purpose whatsoever. Therefore, I waive any kind and all rights I may have for remuneration that could otherwise accrue for using such photos and audio or video recordings.

Registration Fee: $100 per child

Please read before paying:

Complete one registration form for each child. Do not register multiple children on the same form.
The registration fee is $100 per child.
If your child is enrolled in Sacramental Preparation and Middle School Youth Group, you only pay one $100 fee.
If your child is enrolled only in Middle School Youth Group, the fee is still $100.
No child should be charged more than $100.

By Signing you Agree to the following below and the Faith Formation Parent Guide:
- To personally sign your child in and out of the classroom each Sunday. Children may not be dropped off at the curb, in the parking lot, or outside the classroom.
- To bring your child to class regularly and on time and pick them up promptly at 11:45 a.m.
- To attend a Mass other than the 11:00 a.m. Mass on class Sundays so you can pick up your child on time.
- To notify the Faith Formation Office when your child will be absent and understand that frequent absences may affect your child’s readiness to continue in formation or receive the sacraments.
- To help your child bring the required books and materials to class and complete all assigned activities and sacramental-preparation work.
- To actively participate in your child’s faith formation by attending Mass as a family, reviewing assigned work, completing required parent formation classes, and communicating with the Faith Formation Office.
- To ensure your child signs the Mass attendance binder with their grade level and Mass time. Regular Sunday Mass attendance is required as part of Faith Formation and sacramental preparation.
- To understand that preparation for First Reconciliation and First Eucharist is a two-year formation process, but completing two years does not automatically guarantee that your child is ready to receive the sacraments.
- To understand that sacramental readiness will be based on attendance, Mass participation, completed work, parent participation, the child’s understanding of the sacraments, and the child’s desire and readiness to receive them.
- To complete all required parent formation classes and submit all requested certificates, sacramental records, and completed activity books by Sunday, December 27, 2026.
- To inform the Faith Formation Office if your child needs additional support and to cooperate in developing an appropriate plan. If the agreed-upon classroom support plan requires a parent or guardian to remain in class, you agree to provide that support.
- To regularly check email and Flocknote for schedules, reminders, requirements, deadlines, program changes, and other important announcements.
