LONESTAR THUNDER VOLLEYBALL CLUB
CAMP WAIVER, RELEASE OF LIABILITY, AND REFUND POLICY
I certify that my child is physically able to participate in the Lonestar Thunder Volleyball Club camp/clinic and that I am not aware of any injury, illness, medical condition, or other health-related issue that would limit or restrict their participation.
I understand that participation in volleyball activities involves inherent risks, including the possibility of injury. I voluntarily assume all risks associated with my child's participation and accept full responsibility for any injury, illness, or medical expenses that may result.
I agree to release, waive, and hold harmless Lonestar Thunder Volleyball Club, Witherspoon Community Center, its owners, directors, coaches, assistants, staff members, volunteers, and facility representatives from any and all claims, liabilities, damages, or causes of action arising from my child's participation in camp or clinic activities.
I understand that photographs and/or videos may be taken during camp or clinic sessions and may be used by Lonestar Thunder Volleyball Club for promotional purposes, including websites, social media platforms, marketing materials, and other publications. I understand that my child's image may be used; however, names and personal identifying information will not be published without additional consent.
Refund Policy: I understand that camp and clinic fees are non-refundable once the camp or clinic has begun. No pro-rated refunds will be provided for missed sessions, voluntary withdrawal, dismissal, illness, injury, or scheduling conflicts.
