PARENT / GUARDIAN LIABILITY WAIVER, RELEASE & ASSUMPTION OF RISK
THIS FORM MUST BE COMPLETED AND SIGNED BY A PARENT OR LEGAL GUARDIAN
PRIOR TO PARTICIPATION
ACKNOWLEDGMENT & ASSUMPTION OF RISK
I, the undersigned parent or legal guardian, acknowledge that participation in cheerleading, tumbling, stunting, gymnastics-related activities, conditioning, and training involves inherent risks, including but not limited to slips, falls, collisions, strains, sprains, fractures, paralysis, illness, or other serious injury.
I understand that these risks exist even when proper supervision, instruction, and safety precautions are used, and I knowingly and voluntarily allow my child to participate in all Jackpot All-Stars Cheerleading activities.
RELEASE OF LIABILITY & ORDINARY NEGLIGENCE
In consideration of my child being permitted to participate, I hereby knowingly and voluntarily release, waive, and discharge Jackpot cheer, its owners, directors, coaches, instructors, employees, contractors, volunteers, and agents from any and all claims, demands, actions, or causes of action arising from ordinary negligence resulting in injury, illness, or property damage sustained during participation.
This release applies to all activities, whether held indoors or outdoors, including clinics, practices, camps, events, and demonstrations.
This release does not apply to gross negligence or willful misconduct, as prohibited by Michigan law.
INDEMNIFICATION & HOLD HARMLESS AGREEMENT
I agree to indemnify, defend, and hold harmless Jackpot All-Stars Cheerleading, its owners, staff, contractors, volunteers, and agents from any and all claims, damages, losses, or expenses (including attorney fees) arising out of or related to my child’s participation, including claims brought by third parties.
FACILITY & HOST LOCATION RELEASE
I understand that Jackpot All-Stars Cheerleading may operate programs at rented or partnered facilities, including community centers. This waiver also releases the hosting facility, its city or recreation department, and their employees and volunteers from liability related to participation.
EMERGENCY MEDICAL AUTHORIZATION
In the event of an emergency and if I cannot be reached, I authorize Jackpot All-Stars Cheerleading and its representatives to obtain emergency medical treatment for my child. I understand that all medical expenses are my responsibility.

PHOTO & VIDEO RELEASE
I grant permission for my child to be photographed or recorded during participation. I authorize the use of such media for promotional, educational, or marketing purposes, including social media, websites, flyers, and advertisements. I understand that I will not receive compensation and that all media remains the property of Jackpot All-Stars Cheerleading.
