WAIVER
Authorization for Medical Care:
I understand there are risks associated with any program requiring physical activity, and I consent to any emergency medical treatment deemed necessary by the hosting camp (Camp Ghormley) or Slavic Christian Center youth staff (parents will be notified as soon as possible).
I agree that it is my responsibility to obtain my physician's clearance before participating in any physical or strenuous activities. I further understand and agree that the hosting camp (Camp Ghormley) and Slavic Christian Center do not assume any financial responsibility for medical expenses or compensation for any injury that I may suffer during the camp or for any consequences arising from my participation in programs or other activities.
RULES
I agree with the following rules:
· Not to engage in private/secluded meetings between two persons of the opposite gender.
· Bring skirts/dresses (for girls), formal pants, and shirts (for guys) for church services.
· To attend ALL scheduled church and prayer services
· Not to wear inappropriate clothing at any time or anywhere on the campsite. Inappropriate clothing includes the following: shorts, revealing shirts or tops, revealing pants or jeans, low-cut tops, and/or waist-revealing tops (this applies to both genders). Please reach out to the youth leaders if you need further clarification.
· It is advised not to bring valuables, such as expensive electronics or jewelry, as the camp and SCC youth administration will not be liable for your lost, stolen, or damaged property.
· Respect the campsite property, SCC administration, and other campers.
· To drive to and from the camp in a safe and law-abiding manner.
