Waiver and Release of Liability

I, the undersigned parent/legal guardian, understand that participation in basketball and related activities involves inherent risks including, but not limited to, falls, collisions, injuries, illness, or other unforeseen incidents.

I voluntarily allow my child to participate in the UNCAGED Sports Saturday Skills and Scrimmage Mini Camp and assume all risks associated with participation. I hereby release, waive, discharge, and hold harmless UNCAGED Sports, its directors, coaches, volunteers, staff, facility partners, sponsors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of any injury, loss, or damage that may occur during participation in camp activities.

I certify that my child is physically able to participate and authorize league staff to obtain emergency medical treatment if necessary.

I grant permission to UNCAGED Sports to photograph, record video, and use images or likenesses of my child for promotional purposes including social media, flyers, websites, advertisements, livestreams, and future marketing materials without compensation.
