Participant Waiver & Release of Liability
Acknowledgment of Risk
I understand that football training, conditioning, and related athletic activities involve inherent risks, including but not limited to: physical contact, collisions, falls, muscle strains, sprains, fractures, dehydration, and other potential injuries. I acknowledge that participation is voluntary and that these risks cannot be fully eliminated.
Assumption of Responsibility
I voluntarily assume full responsibility for my child’s participation in all training activities, drills, conditioning, and instruction provided by Trench Work / Lineman Training Academy.
Medical Authorization
In the event of an injury or medical emergency, I authorize Coach Kendrick Rogers, Sr. and Trench Work staff to seek emergency medical treatment for my child. I understand that I am responsible for all medical costs associated with such treatment.
Release of Liability
I hereby release, waive, and discharge Trench Work, Lineman Training Academy, Coach Kendrick Rogers, Sr., all staff, volunteers, facility owners, and affiliated organizations from any and all liability, claims, demands, or causes of action arising out of or related to any loss, injury, or damage that may occur during participation in training sessions.
This release applies to injuries caused by negligence, conditions of the training environment, or any other cause, to the fullest extent permitted by law.
Fitness to Participate
I certify that my child is physically able to participate in football training activities and has no medical conditions that would prevent safe participation. I agree to notify the trainer of any relevant medical concerns.
Personal Belongings
I understand that Trench Work / Lineman Training Academy is not responsible for lost, stolen, or damaged personal items.
Parent/Guardian Agreement
By signing below, I acknowledge that I have read and fully understand this waiver and release. I agree to all terms and conditions on behalf of my child.
