Informed Consent and Acknowledgement
I, the undersigned parent/legal guardian of the participant listed below, hereby give permission for my child to participate in the Dwight Howard “Superman” Camp hosted at Border City Sportsplex on Sunday, June 7, 2026.
I understand that participation in sports activities, basketball training, drills, games, and related camp activities involves inherent risks, including but not limited to falls, contact with other participants, physical exertion, and possible injury. I voluntarily assume all risks associated with participation in this event.
I certify that my child is physically able to participate in camp activities and has no medical condition that would prevent safe participation. I understand it is my responsibility to disclose any medical conditions, allergies, or limitations prior to participation.
I hereby release, waive, and hold harmless Border City Sportsplex, Dwight Howard, camp staff, coaches, volunteers, sponsors, and affiliates from any and all liability, claims, demands, injuries, damages, or causes of action arising from participation in the camp, including transportation to and from the event.
In the event of an emergency, I authorize camp staff to obtain medical treatment for my child if I cannot be reached immediately. I understand that any medical expenses incurred are my responsibility.
I also grant permission for Border City Sportsplex and camp organizers to photograph and/or video record my child during camp activities for promotional, marketing, and social media purposes without compensation.
I acknowledge that camp registrations are non-refundable unless otherwise determined by camp organizers.
By signing below, I acknowledge that I have carefully read and fully understand this informed consent and release agreement and voluntarily agree to its terms.
