In consideration of the right to participate in the New Britain PAL Summer Camp, I do hereby, for myself, my heirs, and executors release the New Britain Police Athletic League, the City of New Britain, and all employees or personnel of said parties from any and all liability for property damage, body harm, or any other damages suffered while participating in said Summer Camp and accordingly waive any and all right to bring suit against the New Britain Police Athletic League, the City of New Britain, and employees or personnel.

I ACKNOWLEDGE THAT I HAVE HAD SUFFICIENT OPPORTUNITY TO REVIEW THE PROVISIONS OF THIS DOCUMENT AND UNDERSTAND ITS PURPOSE, MEANING AND INTENT.

PERMISSION TO TRANSPORT

In order for your child to participate in off-site activities while attending PAL Summer Camp, we must have your permission. While participating in summer camp, campers will be transported by bus/van to various locations around the city that include but are not limited to public pools, Hungerford Park, Bee’s Games, New Britain Public Library etc….

I give permission for my child to be transported to off-site activates while in the PAL Summer Camp. I agree to hold the Police Athletic League (PAL) harmless in the event of any injury to my child while he/she is being transported to off-site activities.
