Madison Academy Mustangs Youth Conditioning Camp
Medical Waiver and Release of Liability

I, the undersigned, as the participant or the parent/legal guardian of the participant, consent to participation in the Madison Academy Mustangs Youth Conditioning Camp. I acknowledge that participation in athletic and conditioning activities involves a risk of injury, including but not limited to sprains, fractures, heat-related illness, or more serious injury.

I authorize the Madison Academy Mustangs, its staff, coaches, volunteers, and representatives to act on my behalf in the event of a medical emergency and I assume responsibility for any medical costs incurred.

In consideration of being allowed to participate, I release and hold harmless Madison Academy, the Madison Academy Mustangs, their officers, volunteers, employees, and agents from any liability, claims, or injuries sustained during the camp.

I understand Madison Academy does not provide medical or accident insurance for participants.

Conditioning Camp Parent Code of Conduct

Purpose:
The Madison Academy Youth Football Conditioning Camp is designed to prepare young athletes physically and mentally for the football season while promoting safety, effort, and positive development. This Parent Code of Conduct outlines the expectations for behavior to help ensure a supportive, respectful, and productive environment for all participants.

By signing this agreement, I commit to the following:

1. Support the Camp’s Purpose

I understand that the primary goals of this camp are conditioning, skill development, and team building—not competition.
I will encourage my child to give their best effort and focus on improvement, fitness, and discipline.

 2. Respect for Coaches and Staff

I will support the camp coaches and staff in their efforts to train and motivate all athletes.
I will not question or undermine the coaches’ instructions or decisions during camp.
I will allow the staff to run the sessions without interference.

 3. Positive Behavior and Language

I will model and expect respectful, positive behavior at all times.
I will not use or tolerate abusive, threatening, or inappropriate language or conduct—either in person or online.

 4. Attendance and Punctuality

I will ensure my child attends camp on time and will notify camp staff of any absences.
I understand that consistent attendance is important for safe and effective participation.

 5. Health and Safety Commitment

I will ensure my child comes to camp properly hydrated, dressed appropriately, and ready for physical activity.
I will notify the staff of any medical conditions or injuries that could affect my child’s participation.

 6. Communication and Conduct

I will direct all questions, concerns, or feedback to the camp coordinator or coaching staff in a respectful manner and at the appropriate time.
I will avoid discussing concerns during active sessions and will follow any provided communication procedures.

 7. Sportsmanship and Encouragement

I will encourage my child and others to be positive, work hard, and show respect to teammates and coaches.
I will not compare children or criticize the performance of other participants.

Consequences for Violating this Agreement:
I understand that failure to comply with this Code of Conduct may result in:

A verbal or written warning.
Removal from the camp premises.
Suspension or permanent dismissal of my child from the camp (in severe or repeated cases).

Photo/Video Release: I give permission for photographs/videos taken during the camp to be used for promotional purposes by Madison Academy and the Madison Academy Mustangs.
