HOLD HARMLESS AGREEMENT Ms. Kitschy Witch Art Classes
By enrolling in and/or participating in art classes offered by Ms. Kitschy Witch Art Classes, the participant (or parent/legal guardian if the participant is a minor) agrees to the following terms:
1. Assumption of Risk I understand and acknowledge that participation in art classes may involve the use of various art materials, tools, and equipment, including but not limited to scissors, cutting tools, paints, adhesives, dyes, and other craft supplies. I voluntarily assume all risks associated with participation in these classes.
2. Hold Harmless & Release of Liability I, the undersigned, hereby release, waive, discharge, and agree to hold harmless Ms. Kitschy Witch Art Classes, its owner(s), instructors, employees, volunteers, and agents (collectively, "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, illness, or injury — including death — that may be sustained by the participant while attending or participating in any class, activity, or event offered by Ms. Kitschy Witch Art Classes, whether caused by negligence or otherwise.
3. Medical Authorization In the event of an emergency, I authorize Ms. Kitschy Witch Art Classes and its staff to seek or administer reasonable first aid and, if necessary, to arrange for emergency medical treatment. I understand that I am responsible for all associated medical costs.
4. Allergies & Medical Conditions I agree to notify Ms. Kitschy Witch Art Classes in writing of any known allergies, sensitivities, or medical conditions that may affect the participant's ability to safely participate in class activities prior to enrollment.
5. Photo/Media Release - I grant Ms. Kitschy Witch Art Classes permission to photograph or record the participant during class for use in promotional and social media materials, without compensation.
6. Governing Law This agreement shall be governed by the laws of the state in which the classes are held.
7. Acknowledgment I have read and fully understand this Hold Harmless Agreement. I agree that this release is binding on me, my heirs, assigns, and legal representatives.
