Please complete this form and bring your artwork to the gallery on Intake day ONLY. Artwork Labels will be provided at check in.

ONLY ARTWORK THAT IS LISTED FOR SALE WILL BE UPLOADED TO OUR ONLINE STORE AFTER INTAKE. PLEASE UPLOAD YOUR PHOTOS TO HELP THIS PROCESS GO QUICKLY! THEN REMEMBER TO SHARE OUR WEBSITE SO WE CAN SELL YOUR WORK!

Instructions:
- Use capitalization on your Name and Title of each piece. (NOT ALL CAPS PLEASE!)
- Add the price of your artwork using a $ before your sale amount (ex: $200) or type in NFS (Not For Sale).
- ALL artwork available for sale will be posted on our website STORE so please submit photos taken prior to glass being added to the artwork to eliminate the glare.
- Add the size of your ACTUAL artwork AND the size of the OUTSIDE EDGES of your frame if applicable.
This process works best on a desktop/laptop computer vs cell phone or tablet.
