This is to certify that on this date, I [The Parent/Legal Guardian], as parent or guardian of, [Athlete Participant being registered], or for myself as an adult participant, give my consent to WBS Knights and its medical representative to obtain medical care from any licensed physician, hospital, or clinic for the above mentioned participant, for any injury that could arise from participation in this event.  Initial below to CERTIFY.

WAIVER and RELEASE - By signing below, I understand, acknowledge, and agree that ice skating and ice hockey is an inherently dangerous and physically demanding activity. Participating skaters assume full responsibility for any risk of injury, property damage, or death while participating in activities with our camps, clinics or events. I ACKNOWLEDGE, agree, and represent that I understand the nature of said ice hockey, ice skating and/or physical activity and that I am qualified, in good health, and in proper physical condition to participate in such ACTIVITY. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the ACTIVITY.  Participant and/or their parent(s) or guardian hereby waive any and all claims including, but not limited to, bodily injury including death and property damage they may have against WBS Knights USPHL, LLC, Revo Ice Academy, LLC and its subsidiaries, affiliates, owned or controlled companies, interest owners, co-lessees, contractors, and their officers, directors, partners, employees, volunteers and agents (hereinafter collectively referred to as “WBS Knights, or the “Club”) arising out of Participant's participation in ice hockey camp.   Participants and/or their parent(s) or guardians do hereby release and hold harmless Revo Ice Academy, LLC, WBS Knights USPHL, LLC and its subsidiaries, affiliates, owned or controlled companies, interest owners, co-lessees, contractors and their officers, partners, directors, coaches, employees, volunteers and agents (hereinafter collectively referred to as “Club”) from any and all liability for any loss, damage, or expense that the Participants or their respective heirs, assigns and legal representatives may suffer as a result of Participant's involvement in camp or clinic, from any cause whatsoever, including but not limited to, negligence or breach of contract on the part of the Club in the operation, coaching, supervision, design, maintenance, or condition of the ice surface, or of the ice rink and all surrounding areas.  Participants agree to abide by all rules and regulations, as posted within the ice rink area, for the use of the ice skating rink and will follow the instructions of the operating staff of the rink.  Additionally, this Release and Waiver shall remain in full force and effect in the absence of signs or posted rules or regulations within the rink.  BY USING THIS ONLINE BOOKING FEATURE AND PAYING ANY FEE, I AGREE TO EACH AND EVERY TERM OF THIS RELEASE OF LIABILITY AND WAIVER OF ALL CLAIMS, AND I ACKNOWLEDGE 1) THAT I AM AUTHORIZED TO SIGN ON BEHALF OF THE PARTICIPANTS AND 2) I UNDERSTAND THAT I AM RELEASING REVO ICE ACADEMY, LLC, WBS KNIGHTS AND/ OR  CLUB AS DEFINED ABOVE, FROM ANY POTENTIAL CLAIMS FOR NEGLIGENCE AND FOR ANY DAMAGES SUSTAINED OR CAUSED BY WBS KNIGHTS NEGLIGENCE. I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
