Condition Statements:

Thank you for your interest in the Annual Picnic, including a concert and Museum Open House supporting the Champaign County Historical Society.

There is no rain date for this event. The event will happen rain or shine.

Payment can be made using any major credit card, debit card, ApplePay, GooglePay, or electronic check. Transactions are processed via Square and are encrypted for your protection.

All tickets must be purchased by 11:59pm on June 11, 2026.

Be sure that your browser (particularly Google or Chrome) is up to date. An email is required for online purchase. An email will be sent to you immediately from noreply@jotform.com. Please add noreply@jotform.com to your email contact list.

The event will be held on the grounds at 809 E Lawn Ave, Urbana, Ohio. Dinner will be served at 6:00pm. Concert will start at 6:00pm, and Museum Open House will be immediately following.

1. You may order multiple boxes for each selection. (i.e., three Roast Beef, two Club sandwiches.)
2. Select one of the five box options (a red error warning may appear before you complete the next step).
3. Then choose the side item in the "Side 1" dropdown box.
4. Then select the second side item in the "Side 2" dropdown box.
5. If you order more than 1 of any box, you may substitute the sides on one box by using the "Second box side 1" or "Second box side 2" dropdowns. You do not need to use the second box dropdowns if you only order one.
