I wish for the child named above to take part in the above selected 2026 Gilbert Brown Foundation free football camp(s), held at Champions Riverside Resort in Galesville, WI, benefiting the Gilbert Brown Foundation. I understand that there is a risk of certain changes occurring during or after my participation in an athletic event. In consideration of acceptance of my entry, I do hereby for the child named above, myself, heirs & administrators waive and release any and all claims I have against either Champions Riverside Resort or the Gilbert Brown Foundation. Although I understand that a physician’s exam is not a requirement of participation, it is highly advisable that I consult a physician before allowing my child to participate in any exercise program. I hereby authorize the agents & volunteers of the Football Camp to act in my behalf using their best judgment in any emergency, including medical. I hereby grant the Gilbert Brown Foundation and Champions Riverside Resort, and its football camp volunteers permission to use any photographs or video of my child for promotional purposes.
