I grant permission for my child to participate in the Our Lady of Perpetual Help Catholic Church Faith Formation Program, including Confirmation classes, Youth Group meetings, service projects, retreats/days of recollection, parish events, and other related parish activities.As a parent and/or legal guardian, I remain legally responsible for any personal actions taken by the minor(s) listed above. I agree, on behalf of myself, my child(ren) named herein, our heirs, successors, and assigns, to hold harmless and defend Our Lady of Perpetual Help Parish and the Roman Catholic Bishop of San Diego, and their respective clergy, officers, directors, employees, agents, chaperones, representatives, and volunteers associated with the activities, from any claim arising from or in connection with my minor(s) attending the activities, or in connection with any illness, injury, including death, or cost of medical treatment in connection therewith. I agree to compensate Our Lady of Perpetual Help Parish, its officers, directors, and agents, and the Roman Catholic Bishop of San Diego, and their respective clergy, employees, agents, chaperones, or representatives associated with the activities, for reasonable attorney’s fees and expenses which may be incurred in any action brought against them as a result of such injury or damage, unless such claim arises from the negligence of Our Lady of Perpetual Help Parish or the Roman Catholic Bishop of San Diego. I grant permission for Our Lady of Perpetual Help Parish, its employees, representatives, or volunteers, to photograph or record on audio and/or video, whether tape or digital, my child(ren) for purposes of furthering the mission of Our Lady of Perpetual Help Parish. Photos, audio, or video may be used in printed materials, parish communications, visual displays, social media, websites, or other media sponsored by Our Lady of Perpetual Help Parish. I understand that such photos and/or video recordings will be used for Our Lady of Perpetual Help Parish related purposes only and will not be used for any commercial purpose whatsoever. I therefore waive any and all rights I may have for remuneration of any kind that could otherwise accrue for the use of such photos and/or audio or video recordings. Confirmation Program Requirements: I understand that the Sacrament of Confirmation is part of a two-year formation process at Our Lady of Perpetual Help Parish. This process is meant to help my child grow in faith, participate in the life of the parish community, and prepare to receive the Sacrament of Confirmation. I understand that, in order for my child to successfully complete each year of Confirmation formation and either move on to Confirmation Year Two or be recommended for the Sacrament of Confirmation, my child is expected to fulfill the following requirements: Attend all Confirmation classes. Attendance is essential for my child’s formation. If my child misses a Confirmation class, I understand that the missed class may need to be made up according to the policy of the Confirmation program. Participate in Youth Group meetings. My child is expected to attend a minimum of 10 Youth Group meetings during the formation year. I understand that Youth Group is an important part of the Confirmation process because it helps my child grow in faith, community, prayer, and discipleship. Participate in the Thanksgiving Baskets Project. I understand that my child is expected to participate in this parish service project as part of learning Christian charity, generosity, and service to those in need. Complete 20 hours of community service. I understand that my child is expected to complete 20 hours of service. Service may include participation in parish ministries, liturgical ministries, parish events, Thanksgiving Baskets, Coffee and Donuts with a sponsor, or other approved service opportunities. Attend the Confirmation Retreat or Day of Recollection. I understand that my child is expected to attend the required Confirmation retreat or day of recollection as part of their spiritual preparation for the Sacrament of Confirmation. Participate with their sponsor when required. I understand that the Confirmation sponsor is an important part of my child’s preparation and that my child may be required to attend sponsor meetings, activities, service opportunities, or final project preparation with their sponsor. Complete the final Confirmation project when required. I understand that my child may be required to complete a final Confirmation project as part of the discernment and recommendation process for the Sacrament of Confirmation. I understand that completing these requirements does not mean that Confirmation is automatic. The Sacrament of Confirmation requires proper preparation, participation, and readiness. I understand that the Confirmation team, together with parish leadership, will help discern whether my child is prepared to be recommended for the Sacrament of Confirmation. I also understand that if my child does not complete the required formation, attendance, service, retreat/day of recollection, Youth Group participation, or final project requirements, this may affect my child’s ability to move on to the next year of formation or be recommended for the Sacrament of Confirmation. By signing this agreement, I acknowledge that I have read and understand the expectations of the Our Lady of Perpetual Help Faith Formation and Confirmation Program. I agree to support my child in attending classes, Youth Group, service opportunities, retreats/days of recollection, sponsor meetings, and all required parts of the Confirmation formation process.
