Assumption of Risk & Release of Liability
I acknowledge that participation in athletic activities, including but not limited to volleyball, involves inherent risks, including physical injury, illness, or death.I voluntarily choose to participate in activities organized by Sovereign Sports LLC, including play, officiating, volunteering, setup, and related event activities.I agree to assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation, whether caused by my own actions, the actions of others, or conditions of the facility.I hereby release, waive, and discharge Sovereign Sports LLC, its organizers, volunteers, officials, and any hosting facilities from any and all claims or liability for injury, loss, or damage arising from participation, except in cases of gross negligence or willful misconduct.
Medical & Fitness Acknowledgement
I affirm that I am physically able to participate and have no medical condition that would prevent safe participation. I understand that I am responsible for monitoring my own condition and stopping activity if necessary.
