Liability Waiver
CEL Studios, LLC DBA Bravo Dance Centre reserves the right to drop my student from any and all classes for any reason whatsoever, including but not limited to:
1. Disruptive behavior
2. Excessive tardiness or absence or
3. Non-Payment of accounts
I understand and agree that my student will be participating in a physical activity at Bravo, and as such I realize there is an element of physical risk involved.
I understand and agree that in the event of an accident or illness involving my student, reasonable attempts will be made to notify me immediately. However
in the event that I cannot be reached I hereby designate the emergency contact listed on registration form.
I hereby authorize CEL Studios, LLC DBA Bravo Dance Centre' to take any and all steps it deems necessary to make available and provide medical attention and care, including physicians, hospital, or any other services. Furthermore. CEL Studios. LLC shall have full authority and discretion to make medical and healthcare
decisions for my student. I further agree that the cost of the medical expenses shall be borne exclusively by me. I understand and agree that any limitation in ability to participate due to a medical condition MUST be noted on a separate page and hand-delivered is a warranty by me that no medical conditions exist.
Photo Waiver
I give permission to Bravo Dance Centre (Bravo) to photograph or film my family, and use our likeness in publications created by Bravo or its affiliates. I understand that all images become the property of Bravo. I agree that this material may be published in, but is not limited to, supplemental products, company websites, social media, and advertising materials, and I understand that there will be no compensation. *
