Beginner Crochet - Course Registration

Cost:         $155 Non Member, $145 Member

Photo Release

I grant permission to the Credit Valley Artisans (CVA) to photograph and/or record me and to use these images in CVA publications, newsletters, promotional materials, and social media. I understand that these images may be used without compensation and may remain in use for future promotional purposes.

Policies

• Registration to fill classes will be in the order that fees are received.
• When your fees are received, then you are considered registered.
• Any additional supply fees indicated will be paid to the instructor at
  the first class.
• In the event that the required number of registrants is not reached, we reserve the right to cancel the class up to one day prior to the starting date.
• Payments for classes cancelled by the CVA will be fully refunded.
• If start dates are changed, students will be notified and given the
  option to withdraw from the class. (Unless start is postponed due to weather).
• A full refund less a $20.00 admin. fee will be given to registrants canceling at least 8 days prior to the class start date.
• Please note: Due to the nature of our operation there will be absolutely no refunds given after the 8 day period before the starting date of a class.
• All withdrawals must be made in writing and sent to Course Registrar.

Once your registration form and payment have been received, you will receive a course registration confirmation email.
