Terms and Conditons:
By checking the agreement box and signing below, I acknowledge and agree to the following:
1. I am voluntarily purchasing an at-home DNA testing kit from Precision Diagnostic Testing.
2. I understand that at-home DNA testing kits sold by Precision Diagnostic Testing are intended for informational purposes only unless otherwise specifically identified as legal chain-of-custody testing.
3. I understand that results obtained from an informational at-home DNA test cannot be used in court proceedings, child support matters, immigration cases, inheritance disputes, custody proceedings, or any other legal purpose.
4. I understand that legal DNA testing requires a chain-of-custody collection performed by an authorized collector at an approved collection facility and not at home.
5. I agree to carefully follow all sample collection instructions provided with the testing kit. Improper collection, contamination, insufficient samples, or damaged samples may result in testing delays, additional collection requirements, or additional fees.
6. I certify that I am authorized to provide the submitted DNA samples or that I have obtained appropriate consent from the individual providing the sample. If submitting samples for a minor child, I certify that I am the child’s parent or legal guardian or otherwise have legal authority to consent to testing.
7. I understand that Precision Diagnostic Testing reserves the right to refuse testing if proper consent cannot be verified.
8. I understand that once a test is purchased there are no refunds.
9. I understand that Precision Diagnostic Testing is not responsible for delays caused by shipping carriers, weather events, incomplete paperwork, or insufficient sample quality.
10. I understand that estimated turnaround times are approximate and are not guaranteed.
11. I understand that Precision Diagnostic Testing takes reasonable measures to protect the confidentiality and privacy of all client information and test results.
12. I certify that all information provided by me is true and accurate to the best of my knowledge.
13. I understand that the test being purchased is for 3 participants. The mother, Alleged father and 1 child. For any additional testing supplies to be included you must purchase the additional subject option.
