Participant Acknowledgment & Agreement
In consideration of being permitted to participate in the Activity organized by SETGK, I, the undersigned participant (or parent/legal guardian if the participant is a minor), hereby acknowledge, understand, and agree to the following terms and conditions:
1. Assumption of Risk
I understand and acknowledge that participation in the Activity involves inherent risks, including but not limited to physical exertion, contact with other participants, use of athletic equipment, environmental conditions, travel, and other risks associated with goalkeeper training and athletic participation.These risks may result in injury, illness, permanent disability, paralysis, death, or damage to or loss of personal property. I voluntarily and knowingly assume all such risks, whether known or unknown, foreseeable or unforeseeable, and accept full responsibility for any injuries, damages, or losses sustained as a result of participation in the Activity.
2. Health & Fitness Certification
I certify that the participant is physically and mentally fit to participate in the Activity and has no condition that would prevent safe participation. I acknowledge that it is my responsibility to consult with a qualified medical professional prior to participation if there are any health concerns.I understand that SETGK does not provide medical insurance for participants and that I am solely responsible for any medical expenses incurred as a result of participation.
3. Release of Liability
To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue SET GK LLC, its owners, officers, directors, employees, contractors, coaches, volunteers, sponsors, affiliates, and representatives (collectively, the “Released Parties”) from any and all claims, demands, actions, causes of action, liabilities, or damages, whether known or unknown, arising out of or related to participation in the Activity.This release includes, but is not limited to, claims arising from negligence, premises liability, equipment failure, instruction or supervision, or any other act or omission of the Released Parties.
4. Indemnification & Hold Harmless
I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, liabilities, damages, losses, costs, or expenses (including reasonable attorneys’ fees) arising out of or related to:
- Participation in the Activity
- Any breach of this agreement
- Any act or omission by the participant
5. Media & Photographic Release
I grant SETGK permission to capture and use photographs, video recordings, and other media of the participant during the Activity for promotional, marketing, educational, and commercial purposes, in any medium, without compensation or further notice.
6. Severability & Jurisdiction
If any portion of this Agreement is held invalid or unenforceable under the laws of a particular state, such portion shall be modified to the minimum extent necessary to make it enforceable, and the remaining provisions shall remain in full force and effect.
7. Governing Law & Venue (Multi-State Camps)
This Agreement shall be governed by and construed in accordance with the laws of the state in which the Activity takes place, without regard to conflict of law principles. Any legal action or proceeding arising out of or relating to this Agreement shall be brought exclusively in the state or federal courts located within that state.
8. Out-of-State & Travel Acknowledgment
I acknowledge that the Activity may take place outside my state of residence and may involve travel. I voluntarily assume all risks associated with such travel, lodging, transportation, and participation in the Activity, whether provided by SETGK or third-party vendors.
Acknowledgment & Signature
I acknowledge that I have read this waiver in its entirety, understand its terms, and understand that I am giving up substantial legal rights, including the right to sue. I sign this agreement freely and voluntarily.
