Thank you for your interest in joining Shake The Nations Ministries on our upcoming Gospel Campaign! Please complete the checklist below in its entirety before sending your application along with all the relevant release forms, fees, and documents. It is not our intent for the application and release forms to be cumbersome, but to provide the relevant information we need to protect you as a team member and the ministry as a whole. It’s an honor to take God’s people to see His great power at work on a Gospel Campaign, but this comes with a huge responsibility for you and for us.
BASIC REQUIREMENTS OF AN APPLICANT

1) Be at least 18 years of age. (Or, at least 16 years of age with a parent participating

in the same Gospel Campaign.)

2) Be actively involved in your local church and participate in at least one department

of your local church for at least 6 months.

3) Promptly respond to all e-mail and/or telephone communication from the Shake The Nations

office and/or staff.

4) Adhere to all deadlines, including financial deadlines, regardless of whether the applicant is paying

for the trip themselves or raising financial support to cover the cost.

5) Passport, VISA, vaccinations, and medical prescription costs are the financial responsibility

of the participant and must be obtained in sufficient time before the Gospel Campaign.

BEFORE APPLYING, PLEASE UNDERSTAND THE FOLLOWING:

As a Christian non-profit organization, Shake The Nations Ministries must ensure that its business ethicsand conduct is always founded on mutual respect, integrity, and honesty, based on biblical principles.Shake The Nations Ministries intends to harbor a culture of excellence in all we do to represent Godin the best possible way. Shake The Nations Ministries always operates within the governing laws ofthe country in which we are ministering. I understand that this behavior is required of me as a volunteeron this trip.

I pledge to be prompt and faithful in my attendance to all Shake The Nations commitments.

I pledge to avoid immoral or illegal activities. I will not engage in any behavior that is contrary to biblical conduct and ethics.

FELLOWSHIP
I pledge to be an encouragement to my co-laborers.

FOCUS
I pledge to keep my relationship with God and members of the opposite sex in proper priority.

EXCELLENCE
I pledge to communicate a spirit of excellence in my speech, conduct, appearance and attitude.

LOVING PEOPLE
I pledge to commit to loving the people we serve at STN, which I understand can be done in the following ways:
- Avoiding excess conversation with team members and friends at events
- Concentrate on and serve the people in front of us who are often in need
- Smile and be cheerful
- Have good hygiene
- Speak to people, introduce myself, show a genuine interest in them, and most importantly, listen
- Call people by their names
- Thank them for coming to the event

I understand that all who participate in this Gospel Campaign event are expected to observe and adhere to the following Code of Conduct. As a participant, I understand and agree to the following:
1. I will pray for the Shake The Nations team and give them my undivided attention and support. I will follow their command without criticism or delay. A spirit of unity, care, compassion, and concern for others will guide my thoughts and actions throughout this event.
2. I will adhere to the dress code set forth by Shake The Nations Ministries at all times.
3. I will not leave the team or the vicinity of our work unless directed or allowed by a Shake The Nations leader to do so.
4. I agree that contact with members of the opposite sex should be limited and controlled,so that we reflect the light of Christ and avoid the appearance of inappropriate behavior.
5. I will not participate in activities or visit any establishments that may be seen by others to be inconsistent with a Christian lifestyle.
6. I will not use profanity at any time.
7. I understand that I cannot smoke or consume tobacco products at any time.
8. I will not possess or consume alcoholic beverages or any illegal drug substance.
9. I will abide by any and all additional guidelines set forth by the Shake The Nations Leadership Team.

In consideration for being accepted by Shake The Nations Ministries for a Gospel Campaign, I hereby release, forever discharge, and agree to hold harmless Shake The Nations Ministries and its leaders from any and all liability, claims, or demands for personal injury, sickness, death, property damage, or expenses of any kind that may be incurred by me. I further assume all risks of personal injury, sickness, death, damage, and expenses resulting from my participation in this trip.

I also agree to hold harmless Shake The Nations Ministries and its leaders for any liability arising from negligent, willful, or intentional acts of the participant, including related expenses.

In return for being allowed to participate in volunteer activities and all related or incidental activities (“Volunteer Activities”), I, or my parent/legal guardian if I am under 18, release and agree not to sue Shake The Nations Ministries or its officers, directors, employees, subcontractors, sponsors, agents, and affiliates for any present or future claims made by me, my family, estate, heirs, or assigns for property damage, personal injury, or wrongful death arising from my participation, regardless of when, where, or how it occurs.

I understand and agree that Shake The Nations Ministries is not responsible for any injury or property damage arising from the Volunteer Activities, even if caused by ordinary negligence.

I acknowledge that participation involves risks, including serious injury or death, and I voluntarily accept all such risks.

I also agree to indemnify and hold harmless Shake The Nations Ministries from any claims arising out of my participation.

I understand this document is intended to be as broad and inclusive as permitted by applicable law, and that if any portion is found invalid, the remainder will remain in full force and effect.

I further acknowledge that Shake The Nations Ministries does not provide any insurance for my benefit or that of any related parties, and I represent that I am in good health and have no physical impairments that would prevent my participation in the Volunteer Activities.

PUBLICITY RELEASE:

In return for being allowed to participate in Shake The Nations volunteer activities and all related or incidental activities (“Volunteer Activities”), I, or my parent/legal guardian if I am under 18, grant Shake The Nations and its subsidiaries, affiliates, agents, advertising or promotional agencies, partners, and all associated officers, directors, employees, and successors (collectively, “Authorized Parties”) the absolute and irrevocable right and permission to use, publish, broadcast, and/or copyright my name, address, voice, photograph, likeness, caricature, and personal information in any form, whether original or modified (including being retouched, digitized, cropped, altered, or otherwise changed), in any advertising, promotional, or other materials derived from the Volunteer Activities, in any media now known or later developed, worldwide and in perpetuity, and without compensation. I further agree that any materials created from such use will be owned solely by the Authorized Parties, and I will not permit the use of any copies by any other party. I understand that this document is intended to be as broad and inclusive as permitted by applicable law, and that if any portion is found to be invalid, the remainder will continue in full legal force and effect.

CONFIDENTIALITY AGREEMENT:

While with the Shake The Nations team, you may come across information which is of a confidential nature. By signing the “Team Member Code Of Conduct,” you will be agreeing that you have read these expectations and that you will keep confidential information that you may hear to yourself, which reinforces the pledge of honor we expect from our volunteers. Shake The Nations regards this as pivotal to guarding the integrity of the entire organization and the foundation of its future growth. Volunteers found to breech this contract may be asked to no longer be part of the Shake The Nations Gospel Campaign/ volunteer team.

1. If for some reason, I do not participate on the Gospel Campaign trip to which I have been accepted, I am responsible for any fees incurred as a result of my cancellation.
2. I am responsible for all fund-raising efforts and sponsorships in order to raise money for my Gospel Campaign trip.
3. I understand that if I am not able to raise enough money to cover the cost of my trip, I may not be able to go.
4. If I do not participate on this Gospel Campaign, all materials (including airline and other travel tickets) that have been purchased with donated funds cannot be transferred outside of the trip.
5. If inappropriate behavior, and/or the breaking of any Shake The Nations policies or rules, causes me to be sent home early from the trip as a disciplinary action, no money will be refunded to me.
6. If I am found to break Shake The Nations policies and am sent home early, I will be sent home at my own expense.
7. I understand that Shake The Nations receives and makes all arrangements where a Gospel Campaign is concerned, and I understand that the initial financial amount quoted can change and is not the final amount.
