I, the undersigned parent or legal guardian, hereby grant permission for my child to participate in the AZ Compass Dragon Academy Summer Camp. I recognize that basketball involves physical activity and inherent risks of injury. In consideration of my child’s participation, I voluntarily assume all risks associated with the program and agree to waive, release, and hold harmless Dragon Academy, AZ Compass Prep, Coach Pete Kaffey, and all staff or volunteers from any and all liability, claims, or causes of action arising out of any loss, personal injury, or property damage that may occur during the camp, including transportation to and from the facilities.
Furthermore, I authorize the camp staff to seek and obtain medical treatment for my child in the event of an emergency if I cannot be reached. I certify that my child is in good physical health and has no medical conditions that would prevent safe participation in athletic training. By signing this form, I acknowledge that I have read and understood the terms of this waiver and agree to be legally bound by them for the duration of the program from June 1st through June 4th.
