The Parent/Guardian named above:
- Authorizes their son/daughter named above ("Graduate") to attend the late-night, post-graduation, San Luis Obispo High School Grad Night event ("Grad Night").
- Agrees to assume all risk for injuries or loss arising from their Graduate’s participation in any activity sponsored at Grad Night and to make no claim whatsoever against the Grad Night organizing committee ("Committee"), event sponsors or vendors, or any of the officers, agents or employees of the Committee, event sponsors or vendors, for injuries and/or loss. Understands the Committee provides no medical insurance and will not pay any expenses arising out of any injury and/or loss associated with Grad Night. Authorizes, but does not require or expect, all reasonable first aid, medical and dental treatment, including hospitalization, and will unconditionally pay for any and all such care specified for their Graduate.
Both Graduate/Attendee and Parent/Guardian:
- Agree that Graduate will not carry into the Grad Night premises any materials which are illegal or which will affect another person’s enjoyment of Grad Night.
- Understand that Graduate must be present to win door or raffle prizes.
- Grant release rights without compensation to the Committee to publicly display on social media and in future communications and promotional advertising images of Graduate (no names will be posted) that photographers have captured over the course of Grad Night.
- Understand and acknowledge that Graduate must not engage in the use of alcohol, drugs or other illegal substances prior to or during Grad Night, and understand that Graduate may be subject to a breathalyzer and/or ejection from the Grad Night event if behavior warrants.
- Assume full responsibility for any losses or damages resulting from any Graduate use of alcohol, drugs or illegal substances.
- Understand that once the Graduate enters the Grad Night venue, the Graduate may not leave unless they are released to the Parent/Guardian named above.
Mobile Ops Laser Tag Hold Harmless Agreement:
- In order to participate in Mobile Ops Laser Tag games or activities, I, the undersigned, agree and acknowledge that there is risk of injury from these activities and/or from the equipment involved. I freely assume all such risks, both known and unknown, and assume full responsibility for my participation and safety. I agree to fully comply with all rules and regulations during my participation.
- I, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin hereby release and hold harmless Mobile Ops Laser Tag, their officers, officials, agents or employees, from any and all liability for injury, disability, death, loss or damage to personal property.
- I acknowledge, understand and agree that I have read this release of liability and assume all risk associated with participating and that I sign this release of liability voluntarily and without inducement.
- I hereby certify that I myself, or as the parent or guardian with legal responsibility for the listed participants, agree to my/his/her/their participation and release and hold harmless Mobile Ops Laser Tag, their officers, officials, agents or employees from any and all liability for his/her/their injury, death, loss or damage to personal property.
- If the participant is a minor, I acknowledge that I have read this release of liability in its entirety and assume all risk for the above named minor and sign this release voluntarily and without inducement.
