Agreement for Consulting Services
By submitting this form, I acknowledge that I have read, understand, and agree to the following terms regarding consulting services provided by Anber.
Nature of Services
I understand that these services are strictly consulting in nature and do not constitute psychotherapy, counseling, mental health treatment, diagnosis, or any form of healthcare. These services are not a substitute for licensed mental health care, and I accept full responsibility for obtaining such care independently if needed. I acknowledge and understand that results are not guaranteed.
No Therapeutic Relationship
I understand that participation in this service does not establish a therapist-client, patient-provider, or healthcare relationship.
Professional Boundaries / Dual Relationship Avoidance
I understand that engaging in consulting services is separate from psychotherapy and does not establish a therapeutic relationship. I acknowledge that, due to professional and ethical considerations, it may not be appropriate for me to receive psychotherapy services from Anber now or in the future.
Scope of Services
I understand that all responses are limited to general, educational, and informational insights within a consulting scope. I will not use this service to seek therapy, diagnosis, or treatment recommendations.
Single Question & Format
I agree to submit only one question. I understand that I will receive one response via email within approximately 7 business days and that no ongoing communication or follow-up is included unless I purchase an additional service.
Appropriateness of Service
I confirm that I am not in a crisis or emergency situation and understand that this service is not appropriate for urgent mental health needs.
This service is not intended for crisis or emergency situations; if I am experiencing distress or need immediate support, I agree to call 988, contact local emergency services, or seek care from a licensed mental health provider.
Professional Boundaries and Purpose
I acknowledge and agree that my participation is limited exclusively to consulting purposes. I further agree that I will not seek to establish any personal, therapeutic, or ongoing professional relationship beyond the scope of this service, including, without limitation, engaging in personal inquiries about Anber or otherwise attempting to extend the interaction beyond its intended consulting context.
Client Responsibility
I understand that I am solely responsible for how I interpret and use the information provided and for any decisions or actions I take.
Confidentiality
I understand that, while reasonable efforts are made to protect my information, this service does not provide the same level of confidentiality as a therapeutic relationship. Anber will not reproduce, distribute, or share written responses.
Use of Materials
I agree not to reproduce, distribute, or share written responses without prior written permission from Anber.
Language Requirement
I confirm that I am proficient in English and understand that all services are provided in English. I accept responsibility for any misunderstandings due to language limitations.
Refund Policy
All purchases are final. However, if my question falls outside the scope of this service or cannot be answered for ethical reasons, it may be declined and refunded at Anber’s discretion. This does not guarantee a refund. I agree to review the service description carefully before submitting my question.
Limitation of Liability
To the fullest extent permitted by law, I agree to release and hold harmless Anber from any claims, damages, or liabilities arising from my participation in this service or my use of the information provided.
Severability
If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions will remain in full effect.
Entire Agreement
This document constitutes the entire agreement and supersedes any prior discussions or representations.
Acknowledgment and Consent
By submitting this form, I acknowledge that I have read this Agreement in full, understand its terms, and voluntarily agree to be bound by them.
