As you are entering all four events, your personal details will be shared with the other Host Clubs, to be used for the same purposes as above.

If you are under 18, you need the consent of a parent or guardian to enter the event(s).  We need to collect the name and contact details of the person giving that consent.  This information will only be used in connection with the event(s).

Your parent or guardian will be notified of your entry, and given an option to have your entry deleted if they do not consent.

As you are entering all four events, your parent or guardian details will be shared with the other Host Clubs.

DECLARATION - DMYC Event:
I wish to enter the above boat in the ILCA Sprint Race Event hosted by Dun Laoghaire Motor Yacht Club (DMYC). I agree to be bound by the Racing Rules of Sailing (RRS), the Prescriptions of Irish Sailing, the Rules of the Irish Laser Association (ILCA Ireland), the Notice of Race and the Sailing Instructions. I accept that participation in the Event is at my sole and exclusive risk in every respect.

By this entry I hereby indemnify the Race Committee, the DMYC, its officers, trustees, members, servants and agents in respect of all claims and demands of whatever nature which may be made upon them in connection with or howsoever arising from my participation or intended participation in the Event, and I acknowledge that the Race Committee, the DMYC, its officers, trustees, members, servants and agents accept no responsibility in respect of loss of life, personal injury or loss or damage to property which I or any other person may sustain by participation or intended participation in the Event or howsoever arising in connection with the Event.

I confirm that the boat in which I will participate carries valid third-party liability insurance, with cover of not less than €3,000,000 per incident, to cover my liability in connection with my participation in the Event.

I wish to enter the above boat in the ILCA Sprint Race Events hosted by the clubs listed on page 1 of this form.
