Founding Member & Evergreen Auto-Renewal Agreement:

By enrolling in a Founding or Evergreen package, I agree to the following terms:

1. Discounted Pricing

I understand that I am receiving a discounted rate as a part of agreeing to auto-renewal.

2. Secure Payment Authorization

I authorize Schultz Tennis Academy to securely store my payment method in Stripe, and to charge my card for lesson package renewals as outlined below.

3. Automatic Renewal (Evergreen Program)

My package will auto-renew at the agreed rate, upon 1 lesson remaining, unless I cancel within the designated cancellation window

4. Cancellation Window & Notification

I will receive a notification through my preferred contact point (phone, email) when I have 3 lessons remaining in my package, providing a 2 lesson cancellation window.

5. Refund Policy (Founding Members Only)

If I choose to cancel a package at any time, I am eligible for 25% of the remaining package value, regardless of the amount of lessons remaining.

6. Usage Window

Each lesson package has a 6 month expiration date, unless extended by the Schultz Tennis Academy

7. Non-Transferability

Lessons are non-transferable and may only be used by the registered party.

8. Cancellation Policy

A minimum of 24 hours is the required notice for lesson cancellation or rescheduling, and any lessons cancelled within the window is a risk of forfeiting the lesson fee unless proper proof of a medical or event is provided.

9. Termination of Discounted Rate

Founding & Evergreen member pricing is contingent upon continuous participation in the Evergreen program. If I cancel or allow a lapse in package use, I may lose Founding Member pricing to another.

By enrolling in a Flex package, I acknowledge and agree to the following terms:

1. Refund Policy

Lesson packages are completely non-refundable, unless the Schultz Tennis Academy coach is unable to provide their services.

2. Usage Window

Each lesson package has a 6 month expiration date, unless extended by the Schultz Tennis Academy

3. Cancellation Policy

A minimum of 24 hours is the required notice for lesson cancellation or rescheduling, and any lessons cancelled within the window is a risk of forfeiting the lesson fee unless proper proof of a medical or event is provided.

7. Non-Transferability

Lessons are non-transferable and may only be used by the registered party.

8. Pricing

Lesson and package pricing are subject to change for future purchases, and a purchased rate is not a guaranteed rate for future lessons. Changes will be made known if applicable.

I understand that the Flexible Plan does not include automatic renewal or discounted membership pricing.

Liability Waiver:

Tennis Lesson Liability Waiver & Release of Claims:

I acknowlege that participation in tennis lessons and related activities involves inherent risks, including but not limited to physical injurt, falls, collisions, and other unforseen risks.

I voluntarily agree to assume all risks associated with participating in tennis lessons, training sessions, and any related activities organized by Schultz Tennis Academy.

I herby release, waive, and discharge Schultz Tennis Academy, its owners, coaches, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including serious injury or death, that may be sustained while participating in these activities.

I cerfity that the player is physically fit and have not been advised otherwise by a qualified medical professional. I agree to immediately inform the instructor of any pain or injury during participation.

In the event of an emergency, I authorize Schultz Tennis Academy to both contact the emergency contact and obtain the necessary medical treatment on my behalf if I am unable to do so.

I have read this waiver carefully, fully understand its terms, and sign it freely and voluntarily.
