2026-2027 Faith Formation Registration Form

IMPORTANT NOTES -- PLEASE READ BEFORE REGISTERING:

1. Students are expected to be enrolled in Faith Formation each year from grade 1 through grade 8. We offer several different options for most grades to accomodate families' busy schedules.

2. First Communion is typically received in grade 2, and Confirmation is typically received in grade 8.

3. First Communion prep is a 2-year process, typically beginning in grade 1. However, if your child missed grade 1 class, and you would like them to prepare for First Communion, we can accomodate that. Please reach out to the FF Director (Brenda Terranova, grades1-5@sttheresanreading.org) with questions about the best class placement for your child before registering.

4. 2026 Changes to Faith Formation Offerings:

Students enrolling in First Communion Year 2 must attend class in-person. They can attend a class on Sunday (without parent), or they can attend any of the 5 Family Program classes (with a parent & siblings). This will give families more options for class times for students in First Communion Year 2. See class webpage (linked below) for details.

This year, there will be an earlier 6th grade class option for a total of 3 choices -- Wednesdays at 6 pm, Thursdays at 4 pm, or homeschool for students in grade 6.

5. Confirmation prep is also a 2-year process, typically beginning in grade 7. However, if your child missed grade 7 class, and you would like them to prepare for Confirmation, we can accomodate that. Please reach out to the FF Director (Brenda Terranova, grades1-5@sttheresanreading.org) with questions about the best class placement for your child before registering.

6. The only exception to 2-year sacrament prep is if your student was enrolled in a Catholic grade school for grade 1 or grade 7. If so, they may register directly for Sacrament prep Year 2 in grade 2 or grade 8.

Final Signature

I, as the parent or guardian of the child(ren) I have registered, authorizes the staff and personnel of St. Theresa Parish to treat my son or daughter in case of emergency. Further, I release the staff, personnel, and volunteers of St. Theresa Parish from any act or omission conducted in the course of rendering such care to my son or daughter. In the event of an emergency, your child will be taken to the nearest hospital, unless otherwise notified. In case of a medical emergency, I give permission for my child to be treated by a physician. Please be advised that students' names and/or photos may appear in the parish bulletin, other parish publications, parish social media sites, and/or local newspapers. If you do not give us permission to do so, please contact the Faith Formation office in writing at least two weeks prior to the start of classes.
