By submitting this form, I acknowledge that I am voluntarily enrolling myself and/or my child in programs offered by Fun Station Exploration, powered by Fun Station L.A./I.E. Corporation. I understand that participation may involve physical activity, movement-based fitness activities, interactive games, obstacle courses, and/or hands-on experiences using authentic equipment.
Culinary Arts - Participation & Liability Waiver I understand that participation in culinary activities involves the use of kitchen tools, appliances, and food ingredients, which may present risks including cuts, burns, and allergic reactions. I acknowledge responsibility for disclosing allergies and agree to release and hold harmless Fun Station LE/IE from any liability.
I voluntarily assume all reasonable risks associated with participation and release Fun Station Exploration, Fun Station L.A./I.E. Corporation, its staff, instructors, contractors, partners, and affiliates from liability for injuries, accidents, losses, or damages that may occur during participation, except in cases of gross negligence or willful misconduct.
I agree to follow all program rules, safety instructions, and participation guidelines. I understand that payments, registration fees, and material fees are non-refundable unless otherwise stated in writing.
I also grant permission for photographs and/or videos of myself and/or my child taken during participation to be used for promotional, educational, marketing, and social media purposes by Fun Station Exploration unless I submit a written request opting out.
