I, the undersigned parent/guardian of the above mentioned camper, hereby give permission for my child to participate in the North Crowley Panthers Football Youth Camp (sport/activity) at _North Crowley High School____ (school/organization).

I understand that participation in athletic activities involves inherent risks, including but not limited to physical injury. I acknowledge and accept these risks on behalf of my child.

By signing this form, I agree to release, waive, and hold harmless North Crowley High School/Crowley ISD (school/district), its coaches, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or accident that may occur as a result of my child’s participation in this activity, except in cases of gross negligence.

I certify that my child is physically able to participate and has no medical conditions that would prevent safe involvement. I agree to inform the school of any relevant medical concerns.

In the event of an emergency, I authorize school personnel to obtain medical treatment for my child as deemed necessary.
