Please register to attend our pre-season gym workouts. Included are the waivers necessary for participation. This form is for parents/guardians of athletes ages 17 & under; athletes 18+ may register on their own.

I agree to and will inform my athlete: I/we understand the Crimson Knights Football, Inc. is based on biblical Christian principles the program works to seek, to uphold, and to teach through all of its events. As such, my family including the players and any guests will respect them while attending Crimson Knights events and while representing the Crimson Knights. In this way, I am responsible for all such behavior. Examples of unacceptable behavior include but is not limited to: yelling or gesturing of obscenities from the stands, field, or bench, unsportsmanlike conduct, sexual promiscuity or lewd conduct, inappropriate displays of public affection, disorderly conduct, drunkenness, drug usage, fighting, or stealing, smoking on the property during Crimson Knights events, other non-smoking areas and practice areas, nudity or inappropriate dress. I further understand that in this light, ANY behavior deemed counter to biblical Christian principles that would negatively reflect on the Crimson Knights, or serve as a bad role model to its players could result in disciplinary action including, but not limited to the removal of the family/players from the Crimson Knights program.

I understand and acknowledge participation in a team sport requires each team member to be present at all team activities (training, practice, and games) for the team to collectively put forth the best possible effort. I hereby authorize the Organization to publish photographs taken of me and/or my athlete for use in the Organization's print, online, and video-based marketing materials, as well as other publications of the Organization. I hereby release and hold harmless the Organization from any reasonable expectation of privacy or confidentiality for me and my athlete associated with the images specified above. Further, I attest I am the parent or legal guardian of the athlete and have full authority to consent and authorize the Organization to use their likenesses and names. I further acknowledge participation is voluntary and neither I nor the athlete will receive financial compensation of any type associated with the taking or publication of these photographs or participation in the Organization's marketing materials or other publications of the Organization. I acknowledge and agree publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release the Organization, its contractors, its employees and any third parties involved in the creation or publication of the Organization's publications, from liability for any claims by me or any third party in connection with my or the athlete's participation.

First installment of two payments of $400.00. Your second payment of $400.00 is due 30 days after your first installment. CK staff will follow up with your second invoice via the parent/guardian email provided on this form.

I understand that if my athlete chooses not to play there is only a 50% refund on any registration fees paid before July 6. After July 6 there is NO refund of any fees.

For every sponsor your athlete brings to the Crimson Knights, 50% of the donation goes directly towards their registration fees.
