    **COMPLETE ONE REGISTRATION PER PARTICIPANT**

    Football players must attend one uniform fitting (dates will be announced on our Facebook page.) To ensure timely ordering of the uniform, the registration fee must be received no later than July 15th.

     

    Equipment is the responsibility of the parents/guardians. For football players, this includes, a personal helmet, shoulder pads, cleats, practice pants w/pads, practice jersey and mouth piece. Some items such as helmet and/or shoulder pads may be available to rent for the season but will NOT be guaranteed for each player. Please reach out to our facebook page or email us friendsofthetaftcityparks@gmail.com if you are needing assistance with helmet and/or shoulder pads. 

    All participants are required to participate in fundraising events/activities.

     

    **BIRTH CERTIFICATE, SCHOOL VERFICATION FORM, SPORTS PHYSICAL ARE REQUIRED TO FULLY COMPLETE REGISTRATION**

    Power12welve requires that the registration packet include the player's birth certificate and proof of residence, which can include an utility bill and/or school verfication enrollment form be completed. Proof of residence and/or school verfication enrollment form will be collected at a later date.

    Friends of the Taft City Parks (FOTTCP) is committed to providing a positive, respectful, and supportive environment for all players, families, and volunteers.

    We ask that all parents/guardians demonstrate good sportsmanship and respect toward the organization, board members, volunteers, coaches, players, the Power12 League and its board, and all participating teams and leagues — both in person and on social media.

    If concerns arise, they should be addressed respectfully through the proper channels: first with the head coach, then with a FOTTCP board member and/or the President, rather than through public posts or negative commentary.

    To protect the integrity of our program and those who support it, behavior involving disrespect, negativity, or harmful communication may result in action, including removal from facilities or impact on player participation.

    By submitting this form, you acknowledge and agree to support these expectations and help us keep the focus on a positive experience for all children.

    NONREFUNDABLE DEPOSIT. A nonrefundable deposit may be made to secure your player/cheerleader’s spot. The remaining balance must be paid in full by July 15, 2026. Participants with an outstanding balance after this date will not be eligible to participate.
