Your Permission:

I (the undersigned) give permission for my child / registered participant to attend the Kano Football programs/clinics and will not hold Football Sydney Group and/or Kano Football, its staff, sponsors/partners, or volunteers responsible for damage and or loss of property and or accident and or injury and or death or illness.

I also give permission for an ambulance & medical attention should the need arise.
The parent/guardian gives permission to Kano Football to take photos & videos of the participant for the purpose of promotional material & social media use such as but not limited to Facebook, Instagram, and Twitter.

I the parent also agree not to send my child if:
- They have any cold or flu symptoms
- Have been instructed by NSW/VIC Health to self-isolate.

The Parent/Guardian & player acknowledge all fees are strictly non-refundable. Fees will ONLY be refunded if the application is not accepted. *
