PWRHAUS ELITE CHEER LIABILITY WAIVER & RELEASE

By signing below, I acknowledge that participation in cheerleading, tumbling, dance, and related activities involves inherent risks, including but not limited to injury, permanent disability, or death. I voluntarily assume all risks associated with my child’s participation.

I agree to release, waive, and hold harmless PWRHAUS Elite Cheer, its owners, coaches, staff, and affiliates from any and all liability, claims, or demands arising from injury, illness, or damages incurred during participation.

I certify that my child is physically able to participate and has no medical conditions that would prevent safe involvement.

I understand that PWRHAUS Elite Cheer is not responsible for lost or stolen items.
MEDIA RELEASE

I grant permission for my child’s image, video, and likeness to be used for promotional, marketing, and social media purposes without compensation.
PAYMENT AGREEMENT

I understand that all fees are non-refundable and agree to remain current on all payments. Failure to do so may result in removal from participation.
