General Terms and Conditions for Participation in UMJCA Programs

1. Eligibility

Participation is open to children within the age range listed for the specific program who meet the program’s physical and behavioral requirements. Parents/guardians must complete and sign all registration forms prior to the first session.

2. Program Fees

The total program fee listed, payable in full at the time of registration.

3. Refund Policy

Prorated refunds, minus a $15 cancellation fee, are available if cancellation is requested prior to the second session.

No refunds will be issued after the second session, regardless of attendance.

4. Attendance and Make-Up Policy

Attendance is the responsibility of the participant. Missed sessions will not be refunded or rescheduled.

In the event of program cancellation due to unforeseen circumstances (e.g., weather, facility issues), make-up sessions will be scheduled when possible.

5. Code of Conduct

Participants are expected to follow all program rules and demonstrate good sportsmanship.

Disruptive or unsafe behavior may result in removal from the program without refund.

6. Health and Safety

Parents/guardians must disclose any medical conditions or allergies during registration.

Participants who are ill should not attend sessions.

In case of injury, staff will provide basic first aid and contact the parent/guardian immediately.

8. By participating, parents/guardians consent to the use of photos or videos of their child for promotional purposes. If you do not consent, please notify the program in writing before the first session.

9. Program Modifications

UMJCA reserves the right to modify the schedule, curriculum, or location as needed. Notice will be provided in advance when possible.

Liability Waiver for participation in UMJCA Programs
By enrolling your child in UMJCA programs, you, as the parent or guardian, acknowledge and agree to the following:

1. Assumption of Risk
Participation in sports activities inherently involves physical activity and the risk of injury. By registering your child, you recognize that accidents or injuries can occur, even with proper supervision and precautions. These may include, but are not limited to, sprains, strains, fractures, cuts, bruises, and more serious injuries. You voluntarily assume all risks associated with your child's participation.
2. Waiver and Release of Liability
You hereby waive, release, and discharge UMJCA, its employees, instructors, volunteers, agents, officers, directors, and any affiliates (hereinafter referred to as "Released Parties") from any and all liability for any injury, loss, or damage, including but not limited to personal injury, death, or property damage, that your child may sustain while participating in UMJCA programs, whether caused by negligence, accident, or otherwise, while on or off UMJCA premises, or during activities associated with the program.
Indemnification
3. You agree to indemnify and hold harmless the Released Parties from any and all claims, liabilities, damages, or costs (including reasonable attorney's fees) arising from any injury, damage, or loss to your child or others caused by your child's actions or participation in UMJCA programs.
Emergency Medical Treatment
4. In the event of an injury or emergency, you authorize UMJCA staff to seek emergency medical treatment for your child. You agree to bear any medical costs incurred as a result of such treatment.
5. Acknowledgment of Terms
Signing below, you confirm that you have read and fully understand this Liability waiver. You agree to abide by the terms stated herein and acknowledge that your child's participation is voluntary.
