I, the below signed, release School District 5 and Kalsipell Public schools from all claims on account of injury my child may sustain while on school premise. Signatures are required so School District 5 can continue to keep the premise open for public use. The below signed will not hold Apex Volleyball Academy, its coaches or volunteers responsible in the event of injury, accident or illness.
