With my full understanding of the information above, I agree to assume any and all risks associated with participation with LoCo Fitness. I hereby waive, release, remise, and discharge LoCo Fitness, its agents, partners, employees, and volunteers of any and all liability, claims, demands, actions or rights of actions, or damage of any kind related to, arising from, or in any way connected with my participation in LoCo Fitness events.I give my consent for LoCo Fitness to administer first aid, to call for medical assistance, and have me transported to a medical facility should the need arise. Before signing this document, I have read, understand and hereby agree to the terms and conditions of membership as defined. If I am signing on behalf of a minor child, I also give permission to administer the necessary first aid, and in case of serious illness or injury, I give permission to call for medical and or surgical care for the child and to transport the child to a medical facility deemed necessary for the well-being of the child..I may on occasions be photographed during events at LoCo Fitness. The undersigned hereby consents to the use of photographs, without compensation, on the LoCo Fitness website/social media site or promotional material published by or for LoCo Fitness.
