The Community Outreach Coalition (COC) is excited to have your scholar participate in the Future U camp for Middle and High Schoolers! Dates: *June 8-11, 2026 *June 15-18, 2026 *June 22-25, 2026, *June 29-July 2, 2026. Location: Push Center Time: 9am-2 pm Meals: Lunch and snack will be provided! I have read and accepted this document. Possession or use of alcohol, tobacco, or illegal drugs is a violation of COC. Violation of this policy while on an educational trip will result in the student being sent home at the parent’s expense and other disciplinary consequences upon return to the school. If a student disregards any of the tour rules (ex: leaves the group, breaks nightly curfew, visits in hotel rooms of opposite sex) parents will be notified, and will determine disciplinary action, which could result in a school suspension.
I agree to conduct myself in such a way that I will bring honor to my community, my family, my school, my fellow classmates and myself. I furthermore agree to obey at all times on the tour, the rules of conduct, instructions of the leaders, chaperones or others in authority, and the policies, laws, and regulations, the hotel, conveyances, and facilities visited on the tour. I recognize that any serious violation of rules of conduct may result in my removal from the tour at additional cost to myself or my parents. I will accept the authority of the director and chaperones on this trip and will abide by all decisions they have made and might have to make during the tour.
As a parent, I agree to direct my child to cooperate and follow the directions and instructions of the Community Outreach Coalition personnel. Parent Initials:
Should it be necessary for my child to have medical treatment while participating on this trip, I hereby grant COC personnel permission to utilize their judgment in obtaining medical services deemed necessary and appropriate. I will also grant permission for my child’s photograph, photographic image, and/or video recorded image to be published in materials such as newsletters, annual reports, recruiting presentations, web pages, e-Profiles, etc. I acknowledge and understand the risks associated with trip participation and release this program from any liabilities.
I/We have read the above Student Agreement and have discussed it with my/our son/daughter to make certain he/she understands the importance of proper behavior and the great responsibility of the director and chaperones to see that everything goes well and safely. Participation Commitment:By signing up for any of the Future U Enrichment Camps, I understand that:Participants are expected to attend 100% of the days for each week they register.Each camp builds on the previous day's activities, and full participation is required for the best experience and to earn any certifications (if applicable).I agree to commit to full participation for the week(s) selected.

These images may be used for program promotion and reporting. ⬜ Liability Waiver I understand that while safety is a priority, participation involves inherent risks.
